Click http://tiny.cc/hcsip to fill up the form. All student leaders from Sec 2, Sec 3, Sec 4 and Sec 5 must fill up this form so that your personal records will be updated. Sec 4 and 5 student leaders need not fill in the last page of the form. You may just put N.A. as your answer to all the questions on the last page. *All of you must complete the form by 14th January 2011, 2pm.* Lastly, please do not be late for the meeting.
